The information at the top of the registration form is specific to the child. The bottom section is specific to the parent(s)/legal guardian(s).

Why is Home Phone required? This is the primary phone at which you can be reached, whether that is a land line or a cell phone. It's helpful for us to know in case of emergency. **Do not use parentheses or dashes when entering in your phone number. It will do this automatically.
Can I register a friend? Sorry, no! Only a parent or legal guardian can register their student(s).
What do we do with all of this information? We use all of this information to help place student(s) into small groups, update emergency contact information, etc.
I registered my student last year, do I have to register them again? Yes! As your information changes, we want to make sure that our record of it does too. It's necessary for us to have updated contact info and liability waiver signed for each year.
I can't get the registration to work. What do I do? You should receive an email confirmation upon completion of this registration. If you are having trouble, please contact our Church Registrar at firstname.lastname@example.org
